Funeral Arrangement Sheet
The Funeral Arrangement Sheet serves as the template for collecting all of the information required for planning a funeral. When you arrange with Mount Laurel Home for Funerals, one of our experienced and compassionate directors will use this form to guide you through the process of planning a funeral that is tailored to the specific needs and wishes of you or your loved one. In order to simplify the funeral arrangement process, many people choose to review, fill out, and email us their arrangement sheet. If you are using the Google Chrome or Internet Explorer browsers, you can fill out the form right on your computer.
Organizations That May Require a Death Certificate
This is a list of the various organizations that may require a copy of the death certificate, in order to complete transactions. Some of these organizations will not require a certified copy of the death certificate; a photocopy may suffice. Some organizations may only need to see a certified copy of the death certificate, and they will return the copy to you.
Social Security Form SSA-721
After the passing of a family member, surviving family members may be entitled to benefits from Social Security. This form summarizes these benefits and provides instructions on how to apply for them. More information can be found by calling 1-800-772-1213 or by visiting Social Security’s website.

Veterans of the Burlington County area, and their families, may be entitled to a number of benefits, which are outlined in the above document. When a resident of Burlington County, NJ has passed away, the surviving family members should contact the Burlington County Surrogate, in order to probate the will or set up the administration of the deceased individual. The Outer Burial Container Price List contains a list of vaults and grave liners, as well as information on delivery, of these products, to the cemetery.
